The game is called Excalibur 2555AD. I got the demo version from a PC Gamer CDROM. (Jan 98) There is a demo at http://www.fishuk.com/. I don't think it's exactly the same, but maybe it will work too.
It works with Big RRed "Croc 2". Make sure you rename the file GLIDE2X.DLL, *not* GLIDE2X.VOO. I made screenshots with the PrintScreen key. It is very smooth, I'd say it runs at about 30fps with normal detail. If you turn the lighting details up, probably in the 20's somewhere.
I would love to see textures working. They are probably 8-bit paletted. And the depth buffer seems to be exactly backwards. In other words, we should reverse the depth test. (greater than, less than, etc.) The game is still playable except you can't hear what the characters are saying, nor can you read the text. I have no idea why the sound doesn't work. I am going to try downloading the demo from the internet and see if there is a difference.

These screen shots were taken from Excalibur 2555AD U.S. DEMO,
running in 3Dfx mode on top of Big RRed "Croc 2" and a
Hercules Thriller 3D PCI with 8 Megabytes.
The depth buffering appears to be backward, no textures
are present, and the linear frame buffer access does
not work properly. (The 2D game screens do not appear.)
The colored lighting is present, and the animation is noticably
fluid and smooth, indicating a high frame-per-second count.
